At the login page, you will be asked to create an account- please follow these steps:
Select Create an Account.
Enter your Social Security Number (no dashes, no spaces)
Enter your date of birth in this format: MM DD YYYY.
Enter Access Code: 46640 and Click Continue.
Create your own User ID and password to protect your personal information. Your User ID must contain letters and numbers and be at least 6 characters long. Your User ID and password are both case sensitive.
Answer the remaining questions, then click Submit.
You may now make your coverage selections and designate your beneficiaries.

This is preliminary to the issuance of a booklet and/or online enrollment process explaining the proposed coverage. The Group Universal Life Insurance coverage is issued by The Prudential Insurance Company of America, 751 Broad Street, Newark, NJ 07102. The Booklet-Certificate contains all details, including any policy exclusions, limitations and restriction, which may apply. Contract Series: 96945.
