Note 1. To qualify as a non-smoker, the member/spouse must not have used tobacco or nicotine in any form, including nicotine patches and nicotine chewing gum, within the past 24 months.

Note 2: After you submit your insurance application, the New York Life Insurance Company  will determine if you qualify for Preferred rates. If you meet the Plan's underwriting requirements but are a tobacco / nicotine user, you will automatically fall under the Standard rate category.
